              <h2>About the project</h2>

<h3>NGOMA: Analyzing the rise of non-state actors</h3>

<p>One of the clearest trends in recent history of global health is the <strong>stronger role for non-state actors</strong>, whether corporations, philanthropic foundations, public-private partnerships or non-governmental organisations and civil society groups.</p>

<p>The NGOMA project addresses this trend by <strong>examining the role of health-related NGOs in ‘transferring’ or diffusing global norms, policies and programmes </strong>targeting women’s and adolescent’s health to the national health and education system and local communities in Malawi.</p>

<p>The project has developed a unique research focus on the role of such actors, applying critical ethnographic methods to understand their impact on policy, governance and practice. Through a <strong>multi-sited ethnographic study</strong>, the project examines how policies and programmes made by global communities of practice work themselves out, and to what effect, within national policy circles and local practice.</p>

<h3>Maternal Health: a field increasingly relying on NGOs</h3>

<p>The maternal health field has recently been going through a rapid policy shift. At the end of the MDG era, the focus in maternal health was primarily on skilled birth attendance. At the end of 2014, the policy discourse had narrowed down to teenage pregnancies and how to prevent them – through keeping girls in school. The <strong>influx of NGOs in health development</strong> enabled this profound rapid shift, by bypassing the time consuming and bureaucratic process of going through national policy channels.</p>

<p>NGOs, on the other hand, as channels for these global norms are faced with the reality of <strong>competing for short-term funding</strong>, within an increasingly competitive environment. NGOs find themselves under constant pressure to produce success, which <strong>hinders a more critical view of learning </strong>and the fault lines in the implementation process.</p>

<p>This was further intensified when the newly elected President Trump reinstated and expanded the Mexico City Policy, or the <strong>global gag rule</strong>, banning foreign NGOs from providing abortion services, lobbying for abortion or even informing women about abortion.</p>

<p>This lead to a further <strong>shrinking of the political space for civil society </strong>that heavily affects the ability of civil society to have a distinct political voice. In local communities, the vast number of NGOs implementing all kinds of small-scale projects is met with distrust and resistance.&nbsp;</p>

<h3>The changing role of NGOs</h3>

<p>NGOs are at present <strong>unable to voice needs of communities</strong>, but are <strong>instruments for the implementation of global policies</strong>. As such, they have lost the profound role of civil society as intermediaries between policy makers, be they governmental or global, and local communities.</p>

<p>The fact that donors bypass governmental structures, favoring NGOs as implementing actors, further weakens already fragile national health and education systems.</p>

<h3>Research outcomes</h3>

<p>The NGOMA project has clear policy relevance, given that bilateral donors, including Norway, direct substantial funding to non-governmental organisations, primarily international NGOs. Today, the information of these implementing practices is produced by NGOs themselves that is heavily limited by a strict reporting system and the pressure to produce success. The NGOMA project is very timely as we are at the forefront in describing and analyzing these profound changes in the health development field.</p>

<p>The NGOMA project takes note of this timeliness by publishing in leading social science and public health journals in order to engage both the social sciences as applied to health, and public health research, policy and practice.</p>

<p>In addition, the project is committed to knowledge exchange beyond the academic sphere, by providing scientific input at policy-related conferences, panels of donors and nongovernmental organisations and popular press. &nbsp;</p>

<h2>Financing</h2>

<p>The project is funded by the Research Council of Norway’s GLOBVAC Programme (2014-2018).</p>

<h2>Cooperation</h2>

<p>The project is a collaboration between the Centre for Development and the Environment at the University of Oslo and Chancellor College at University of Malawi.</p>

                            <p class="vrtx-publication-summary" style="display:none">In March 2017, Save the Children International complied with the reinstated Mexico City Policy, banning US funding to foreign non-governmental organizations (NGOs) that inform about, advocate for, or provide abortion services.
Several of its member organizations, among them Save the Children Norway, strongly opposed this decision, arguing that reproductive health is not solely about health but also concerns rights. This article explores how Save the Children’s pragmatic choice of going global and becoming an international NGO creates various ideological dilemmas for Save the Children Norway, challenging its identity as a civil society actor. It draws on fieldwork conducted within Save the Children Norway 2014–2017, to identify some internal and ideological dilemmas faced by Save the Children Norway and staff as they try to preserve their autonomy and identity while being part of a one of the world’s largest NGOs – Save the Children International.</p>

                            <p class="vrtx-publication-summary" style="display:none">Global health donors increasingly embrace international non-governmental organisations (INGOs) as partners, often relying on them to conduct political advocacy in recipient countries, especially in controversial policy domains like reproductive health. Although INGOs are the primary recipients of donor funding, they are expected to work through national affiliates or counterparts to enable ‘locally-led’ change. Using prospective policy analysis and ethnographic evidence, this paper examines how donor-funded INGOs have influenced the restrictive policy environments for safe abortion and family planning in South Sudan and Malawi. While external actors themselves emphasise the technical nature of their involvement, the paper analyses them as instrumental political actors who strategically broker alliances and resources to shape policy, often working ‘behind the scenes’ to manage the challenging circumstances they operate under. Consequently, their agency and power are hidden through various practices of effacement or concealment. These practices may be necessary to rationalise the tensions inherent in delivering a global programme with the goal of inducing locally-led change in a highly controversial policy domain, but they also risk inciting suspicion and foreign-national tensions.</p>

                            <p class="vrtx-publication-summary" style="display:none">As intermediaries between donors and beneficiaries ‘local Development brokers’ play a crucial role in shaping the implementation of development initiatives. They tie together different interests through acts of translation and organise
development interfaces, but also pursue their own ambitions. This article examines junior public servants in Malawi’s primary education sector, who as a result of shifting aid modalities and priorities, have become development brokers in the implementation of multiple non-governmental organisation (NGO) projects. Studying their various ‘broker’ roles provides an analytical lens through which to examine the active co-construction of development initiatives, and how brokering affects their position and the school as a public institution. The analysis is based on ethnographic fieldwork at an under-resourced primary school in Mangochi District and on the implementation of norm-promoting projects aiming to keep girls in school and reduce teenage pregnancies. This article describes how brokers facilitate
NGO activities, translate global norms into messages that resonate locally, and strategically present successes in line with project discourses. This article argues that
these strategies are intended to sustain the projects to benefit the school, the students and to supplement low salaries, thereby prioritising short-term benefits over the quality of education. Donors’ and INGOs’ well-intentioned efforts to strengthen country systems, might result in undermining broader educational goals if these attempts come in the form of multiple small-scale NGO projects. These critical reflections do not travel up the aid chain, as brokers are incentivised to produce successes.</p>

                            <p class="vrtx-publication-summary" style="display:none">Despite the strong global focus on improving maternal health during past decades, there is still a long way to go to ensure equitable access to services and quality of care for women and girls around the world. To understand widely acknowledged inequities and policy-to-practice gaps in maternal health, we must critically analyse the workings of power in policy and health systems. This paper analyses power dynamics at play in the implementation of maternal health policies in rural Malawi, a country with one of the world’s highest burdens of maternal mortality. Specifically, we analyse Malawi’s recent experience with the temporary reintroduction of user-fees for maternity services as a response to the suspension of donor funding, a shift in political leadership and priorities, and unstable service contracts between the government and its implementing partner, the Christian Health Association of Malawi. Based on ethnographic research conducted in 2015/16, the article describes the perceptions and experiences of policy implementation among various local actors (health workers, village heads and women). The way in which maternity services “fall apart” and are “fixed” is the result of dynamic interactions between policy and webs of accountability. Policies meet with a cascade of dynamic responses, which ultimately result in the exclusion of the most vulnerable rural women from maternity care services, against the aims of global and national safe motherhood policies.</p>

                            <p class="vrtx-publication-summary" style="display:none">In July 2015, Malawi’s Special Law Commission on the Review of the Law on Abortion released a draft Termination of Pregnancy bill. If approved by Parliament, it will liberalize Malawi’s strict abortion law, expanding the grounds for safe abortion and representing an important step toward safer abortion in Malawi. Drawing on prospective policy analysis (2013–2017), we identify factors that helped generate political will to address unsafe abortion. Notably, we show that transnational influences and domestic advocacy converged to make unsafe abortion a political issue in Malawi and to make abortion law reform a possibility. Since the 1980s, international actors have promoted global norms and provided financial and technical resources to advance ideas about women’s reproductive health and rights and to support research on unsafe abortion. Meanwhile, domestic coalitions of actors and policy champions have mobilized new national evidence on the magnitude, costs, and public health impacts of unsafe abortion, framing action on unsafe abortion as part of a broader imperative to address Malawi’s high level of maternal mortality. Although these efforts have generated substantial support for abortion law reform, an ongoing backlash from the international anti-choice movement has gained momentum by appealing to religious and nationalist values. Passage of the bill also antagonizes the United States’ development work in Malawi due to US policies prohibiting the funding of safe abortion. This threatens existing political will and renders the outcome of the legal review uncertain.</p>
